2021 -22 Scholarship Application

GOAL

The goal of the scholarship program is to provide educational funding to support students of Middleton Senior High School, which will assist applicants in achieving lifelong educational goals. 

SCHOLARSHIP AWARD

The scholarship is awarded each year based on availability of funds. The awards are presented for academic excellence, community involvement, character, and demonstrated leadership qualities and responsibilities. The scholarship is awarded after a competitive screening process. The scholarship will only be distributed to the selected students upon proof of enrollment at an accredited college, trade or technical school. 

REQUIREMENTS:

1. Applicant must be a student in their senior year at Middleton Senior High School.

2. Applicant must provide proof of enrollment or acceptance in an accredited post-secondary college/university of other professional training institution.

3. Applicant must be a citizen of Hillsborough County, Florida, and permanent resident of the United States.

4. Applicant must have a 2.5 minimum grade point average (GPA) at time of application and must be verified by an official transcript or certifying letter from the appropriate school official.

5. Applicant must submit a minimum 500-word essay stating how and why he/she chose their field of study, and how this scholarship would benefit them in the pursuit of higher education. Please incorporate your experiences, goals, and aspirations into the essay. ALL ESSAYS SHOULD BE TYPED, DOUBLE SPACED, and USE A 12 POINT Arial or Times New Roman Font. 

6. Applicant must submit one non-relative reference.  Reference should be made on letterhead of official organizations. Examples are schoolteachers, church leaders, community leaders, employers, supervisors, etc.    

7. Please list your outside activities which show your involvement in civic community activities over the past two-three years, which may include work experience during your high school years, as well as the number of documented community service hours (a minimum of 100 hours required). 

8. Please upload with the application, a recent wallet size (2 x 3 inches) photo - graduation photo is acceptable.

9. Application must be postmarked by April 22, 2022, to be considered.

The Scholarship Committee will review all applications and notify applicants of their status. All winners will be notified by mail.

Scholarship Application

Application must be completed by April 22, 2022. Please ensure that you review the requirements and thoroughly, complete this application and upload all DOCUMENTS. If information is missing, credit will not be given during the rating process. This scholarship is for students of Middleton High School who are or will be pursuing a higher level of education. The recipient may be a graduate of public secondary education, special/adult education programs or a government financed program.
